An education document (diploma) can be reissued only in the following cases:

• Loss (misplacement, destruction, etc.),

• Damage,

• Presence of errors,

• Change of gender of the individual to whom the education document was issued.

When receiving a duplicate diploma, a duplicate of the diploma supplement or an archival certificate must also be issued.

List of Documents Required for the Restoration of Education Documents: 

• Application form

• Questionnaire

• Consent for personal data processing

• Copy of the passport

• Copy of the birth certificate, marriage/divorce certificate (if the surname has changed)

• Copy of the identification code

• Copy of the lost diploma

• Newspaper announcement stating that the diploma should be considered invalid

• Police certificate confirming the acceptance of a loss report

• Copy of the foreign passport

• Payment receipt (payment details are provided upon submission of the document package)

If a foreign passport is not available, the transliteration of the full name is carried out in accordance with Resolution No. 55 of January 27, 2010, On the Standardization of the Transliteration of the Ukrainian Alphabet into Latin Script (as amended on January 12, 2016).
